In Word 2003:
Goto Tools > Options > View Tab ...and uncheck the one that says Picture Placeholders.
Works for some.
As far as format there is no unique format. The images are in PNG format and the doc is simply saved as a 2003 document. When I get it all completed I plan to publish to PDF so it can be better printed and bound as an actual book.

In light of the the mini "FU" SE has thrown down in this last round of questions to all us DoL'ers...it appears they might be jacking with the bodies of water.
Instead of giving us:
* New fish/legendary fish
* New gathering quests
* Improved Gathering fun/party gathering
They basically said, "Yeah no DoL is a solo only affair and all those grade 6-10 waters will likely be turned into Grade 1 since that' all we put in the waters was low level fish"
Not only does this mess with the leveling of Fishing, but makes this guide be put on hold for a moment until they decide what they are doing. I'll still be adding maps and bodies of water for Grades 1-5 but the higher rank grades will sit idol for a bit untill the sort it all out. (dont want to have to redo all my maps >.>)
